Viewing Customer-Uploaded Documents in Disc Image

Thanks to the OK Benefits customer portal, customers can now digitally upload a variety of documents for their case worker(s). This article will describe the types of documents that can be uploaded, and demonstrate how to view them in Disc Image.

There are currently several ways a customer can upload documents:

  • When submitting an application for services, customer can upload additional documentation (Divorce Decrees, Birth Certificates, Tribal Membership Cards, etc.)
  • By selecting an Associated Document, customer can complete and submit a CSS form. There are currently only two documents available – the Address of Record and Family Violence Statement, and the Affidavit of Payment (CP and NCP version)
  • By updating information through the Report A Change feature on the portal
  • By submitting new address or employer information for the NCP
  • By using “My Uploads” to submit one or more documents

The following types of documents will be available to view on Disc Image:

In Disc Image, CSS applications can be viewed in the “CSS Applications” folder. Other documents can be found in the “Customer Uploads” folder.

You can search by various indices (document date, document type, FGN, etc.) by selecting one from the first “Fields” drop-down and clicking “Search”.

It is important to note that a customer may mislabel a document – for example, they may select that the document is a Divorce Decree rather than a Marriage License.
