What is Personally Identifiable Information (PII)?

Personally identifiable information (PII) is information that, when used alone or with other relevant data, can identify an individual.

Per the State of Oklahoma PII breach notification law, PII includes:

  • First and last name
  • First initial and last name in combination with:
  • Social Security Number
  • Driver’s License Number
  • Bank Account Number
  • Credit Card Number
  • When used in conjunction with other identifying information:
  • Date of Birth
  • Address
  • DCN and FGN

Per IRS Publication 1075, PII is FTI when provided by the IRS. When in doubt ask yourself:  where did this information come from? If the answer is the IRS then it is FTI.
