Electronic Access Registration Form for Bigger Stamp

OAC 340:2-28-4.2 Terms and location of Office of Administrative Hearings: Child Support

The Electronic Access Registration form shall be used to register for an account on the Office of Administrative Hearing’s (OAH) Court Electronic Filing System for Child Support issues.  Only those registered with the Court’s system will be able to electronically file documents and/or view case information.

Use of Form

Please use this form when onboarding new staff and also for those who transfer from another office. This is for staff who use OAH Workflow and OAH Case Manager.

The following information is required to register with the Court’s system.

  • Last Name, First Name, MI
  • E-mail Address
  • Phone No
  • Role (CSS Admin, CSS Caseworker, CSS Supervisor, CSS Attorney, OAH Judge, OAH Clerk)
  • User ID
  • Date
  • Signature
  • Notary stamp or seal (for Notaries)

When you have completed the form, please scan it and email it to to the OAH Legal Staff.
