How to Request an OAH Case Number

To request an OAH Case Number:

1. Go to the Home Tab and Click on Forms.

2. Click on 830 OAH New OAH Number Request Form.

3. Fill in the FGN and any Notes/Comments.

4. Click the Submit button.

Within a few minutes you will receive an email with the new OAH case number.
