Notice Requirements in UIFSA

States are required to use the federally approved standard intergovernmental forms in intergovernmental actions.

The list of revised intergovernmental forms below is available on the Intergovernmental Child Support Enforcement Forms page of the OCSE website.

Caseworkers and Child Support staff should continue to use the forms available in ALC. Staff should use the forms on the OCSE website for reference or in situations when ALC generated documents are not available.

There are two more forms that came out in 2021 (to expire June 20, 2024). These last two forms listed below are not found on the Intergovernmental Child Support Enforcement Forms page.

(For processing international cases under the Hague Convention, please see alternative forms on OCSE’s Hague Convention forms webpage. For processing cases with Foreign Reciprocating Countries that are not Hague countries, see the appropriate Caseworker Guide on the International webpage.)
