Caseworkers and Child Support staff should continue to use the forms available in ALC. Staff should use the forms on the OCSS website for reference or in situations when ALC generated documents are not available.

- Child Support Enforcement Transmittal #1 – Initial Request
- Child Support Enforcement Transmittal #1 – Acknowledgment

- Child Support Enforcement Transmittal #3 – Request for Assistance/Discovery
- Uniform Support Petition
There are two more forms that came out in 2021 (to expire June 20, 2024). These last two forms listed below are not found on the Intergovernmental Child Support Enforcement Forms page.
