Notice Requirements in UIFSA

States are required to use the federally approved standard intergovernmental forms in intergovernmental actions.

The list of revised intergovernmental forms below is available on the Intergovernmental Child Support Enforcement Forms page of the OCSE website, as well as being linked below. These standard forms are due to expire December 31, 2022.

There are two more forms that came out in 2021 (to expire June 20, 2024). These last two forms listed below are not found on the Intergovernmental Child Support Enforcement Forms page.
