Obtaining an Acknowledgment of Paternity Form

The Acknowledgment of Paternity (AOP) form may be obtained in several different ways in the state of Oklahoma.

1. Birthing facilities
   1. The preferred and most common way to obtain an AOP form is at the birthing facility. All birthing facilities in the state of Oklahoma participate in the Acknowledgment of Paternity program.
