Completion and Execution of Acknowledgment of Paternity Form

Once the information on the Acknowledgment of Paternity (AOP) form is completed, the form must be signed by the biological mother and father and witnessed by at least one non-related party.

1. Each signature must be witnessed.
2. The signing parties cannot witness each other’s signature.
3. The mother’s and father’s signatures can be witnessed by the same person as long as that person is not related to either signatory.
4. The mother’s signature and the father’s signature can be witnessed by different people as long as those individuals are not related to either signatory.
