Making a Child Welfare or Adult Protective Services Report

When concerns arise regarding the safety or well-being of a child or vulnerable adult, it is critical to take timely and appropriate action. Staff are responsible for ensuring reports are made accurately, thoroughly, and with care.

To initiate a report, follow the steps below:

  1. Complete the Required Reporting Form
    Access the CPS/APS Reporting Form located on InfoNet under Quick Links. Ensure all required fields are completed with as much relevant and factual information as possible. Be objective and avoid assumptions, document only what is known or has been disclosed. You may reach out to the PATHS team at *OCSS.PFRs for guidance or support.
  2. Contact the Hotline
    After completing the form, immediately contact or email the hotline to make the report. Be prepared to provide the information documented on the form and answer any follow-up questions. Ensure you receive the referral number from your report. When making referrals, you may elect to remain anonymous.
  3. Document in the Case Log
    Once the report has been made, update the case log to reflect the action taken. Reference the referral number provided.Example Caselog: “Internal Use Only/Do Not Release to customer Child Welfare referral made on this date due to concerns of: (domestic violence, child abuse, child neglect, etc.). CW Referral Number: XXXXXX.” Keep documentation concise and professional.
  4. Submit the Completed Form
    Send the completed reporting form to the PFR email inbox at *OCSS.PFRs for tracking and documentation purposes.

Taking these steps ensures concerns are reported consistently and responsibly, while maintaining professionalism and sensitivity.
