What happens when a “Good Cause” claim is denied?

When the customer is denied good cause to cooperate, the C-12 form is generated to inform the customer of the decision. This document informs the customer they may request a review and to provide additional information to support the good cause claim. Once completed the form is returned to State Office Domestic Violence Unit for review and final good cause determination. The Domestic Violence Unit will inform the customer and district office of the decision.
