OAH Hearing Request Form

The OAH hearing request form on OnBase is for contested hearings only. Docketing or Signature dockets are a separate process.

To request a hearing with OAH on court day:

1. From the Home page of OnBase access the form by clicking on the Forms button on the ribbon.

2. Click on 830 CSS OAH Hearing Request Form.

3. Enter the OAH Court Number and press enter. The FGN, NCP name, and CP name will auto fill.

4. Complete the form. The CSS Exhibits Submitted to OAH?, Point of Contact, State’s Attorney Name, and NCP/CP Phone Number fields must be completed, the other fields are optional.

5. Click the Submit button.
