What forms should be received on Incoming Intergovernmental cases?

The Interstate Central Registry (ICR) will receive the documents for new incoming intergovernmental cases. The ICR will review the documents and verify that all the documents needed to process the request are received. If not all documents are received they will request them from the initiating state.

Incoming Parentage/ Establishment cases should include the following documents:

  • Transmittal #1
  • Child Support Agency Confidential Information
  • Declaration in Support of Establishing Parentage (only if parentage is at issue in the case)
  • Uniform Support Petition
  • Personal Information Form
  • General Testimony
  • Supporting Documents

Incoming Enforcement cases should include the following documents:

  • Transmittal #1 Initial Request
  • Child Support Agency Confidential Information
  • Letter of Transmittal Requesting Registration (only if the order is from a state other than Oklahoma)
  • Certified copy of the order
  • Updated Arrears Computation

Incoming Modification request should include the following documents:

  • Transmittal #1
  • Child Support Agency Confidential Information
  • Letter of Transmittal Requesting Registration (only if the order is from a state other than Oklahoma)
  • Uniform Support Petition
  • Personal Information Form
  • General Testimony
  • Supporting Documentation
