Designation of Authorized Representative Form

The Designation of Authorized Representative (G07) form allows a customer to name one person, age 18 or older, to act as their authorized representative on their child support case.

What the Authorized Representative can do: 

  • Receive information about the child support case
  • Give information on the customer’s behalf

 What the Authorized Representative cannot do:

  • They cannot appear in court in place of the customer or sign documents on their behalf 
  • If the customer does not appear, the court may still enter an order without their input

 Customers Under 18: 

If the customer is under 18 years of age:

  • They must designate an authorized representative
  • They must appear in court with their authorized representative, parent, or next friend 
  • The authorized representative will continue to have access to case information after the customer turns 18, unless the designation is revoked or canceled in writing or by completing a new G07 form

 Important: 

  • Only one authorized representative may be designated at a time
  • The designation may be revoked or canceled at any time in writing or by submitting a new G07
  • Until it is revoked or canceled, the authorized representative will continue to have access to case information
  • CSS reserves the right to refuse to honor a designation of an authorized representative
  • When CIC opens a case or receives a new referral on an existing case, the G07 will be sent only for cases involving minor parents. CIC will not send the G07 for all cases.
