If you have a case where you are locating a Custodial or Non-Custodial parent, make a request by completing the CLEAR Search Request form.
Each person you need a CLEAR search on must be separate request form filed out and even if your office has a designated CLEAR person, you must file out this form to account for tracking of these requests.
