Required Documents for Outgoing Intergovernmental Cases

To initiate an outgoing referral for paternity/establishment the following documents are needed:

1. Transmittal #1 (IT1E)
2. Child Support Agency Confidential Information Form ( ITCIE)
3. Declaration in Support of Establishing Parentage (only for paternity cases) (I16F)
4. Uniform Support Petition (I17F)
5. Personal Information Form for UIFSA 311 (I35)
6. General Testimony (I18F)
7. Any supporting documentation (such as birth certificates, pay stubs, tax records, etc.)

To initiate an outgoing referral for enforcement the following documents are needed:

1. Transmittal #1(IT1)
2. Child Support Confidential Information Form (ITCI)
3. Letter of Transmittal Requesting Registration (if the order is not from the responding state) (I20)
4. Certified copy of all orders (If the order is not from the responding state) – “certified copy” reflects “true and correct” copies
5. Updated Arrears Computation
6. Any supporting documents (such as pictures of the NCP, paystubs, taxes, license information, etc.)

To initiate an outgoing referral for a modification the following documents are needed:

1. Transmittal #1 (IT1)
2. Child Support Confidential Information Form (ITCI)
3. Letter of Transmittal Requesting Registration (if the order is not from the responding state)
4. Uniform Support Petition (I17F)
5. Personal Information Form for UIFSA 311 (I35)
6. General Testimony (I18F)
7. Any supporting documentation (such as paystubs, cost of insurance, daycare cost, etc.)
