This article explains the different ways of transmittal of the Acknowledgment of Paternity form to Oklahoma State Department of Health Division of Vital Records.
- Acknowledgment of Paternity (AOP) obtained at birthing facility
- When completing the birth certificate packet, birthing facility personnel explain the rights and responsibilities of acknowledging paternity and provide the AOP form for completion if the parties are interested.
- After the AOP is completed and executed at the birthing facility, it is transmitted by the birthing facility to Oklahoma State Department of Health (OSDH) Division of Vital Records.
- The AOP form does not have to be completed at the birthing facility. It can be completed by the mother and father at any time.
- AOP obtained at OSDH Division of Vital Records
- If the AOP form is obtained from OSDH Division of Vital Records, it can be completed and executed onsite.
- If the AOP form is completed and executed at OSDH Division of Vital Records, the form is then given to OSDH Division of Vital Records personnel for processing.
- Although the AOP may be obtained from OSDH Division of Vital Records, it does not have to be completed at OSDH Division of Vital Records.
- AOP obtained through other means
- If the AOP form is obtained at an OSDH County Health Department, a Child Support Services (CSS) District Office, a local Oklahoma Department of Human Services (DHS) office, or online at the DHS website, the mother and/or the father is/are responsible for the proper execution of the form and its transmittal to OSDH Division of Vital Records.
